DIRECT PRIMARY CARE:

Children (0-18)   -   $30/month

Adult (18+)           - $80/month

$99 registration fee per patient, waived for additional family members who sign up at the same time and live at the same address. If you cancel your membership and decide to rejoin at a later time, you will be required to pay the registration fee again.

Note:  Children must enroll with a parent/guardian.  Special pricing available for employers enrolling 5+ employees.

COSMETIC AND AESTHETIC SERVICE PRICING:

Prices vary by treatment.   Membership not required for these services.

   

DOT EXAMS:

$150 includes the exam and completion of required forms, as well as 1 follow up visit if needed, during our regular business hours. After hours exams may be arranged for an additional fee to accommodate those with inflexible work hours. Membership not required for this service, but can be rolled into a Direct Primary Care relationship.

MASSAGE THERAPY:

Massage pricing is as follows:

 50 minutes  $ 65  

 80 minutes  $ 95

110 minutes $120

Membership is not required for this service, however members receive a discount on the prices listed above.
